Below is a list of crime investigations and activities reported by the Forsyth County Sheriff’s Department in Clemmons and Lewisville last week.

• Dec. 10 — police service-assist agency, 1100 block of South Peace Haven Road, Clemmons. Medical staff reported that a resident had been assaulted.

• Dec. 10 — lost property, 6800 block of Shallowford Road, Lewisville. Man reported that he lost his wallet at the grocery store.

• Dec. 11 — traffic accident, 4100 block of Clemmons Road, Clemmons. Three-vehicle accident with damages of $5,500, $3,000 and $500.

• Dec. 11 — traffic accident, 6100 block of Stadium Drive/Ridgecrest Drive, Clemmons. Two-vehicle accident with damage of $200 to one vehicle.

• Dec. 11 — traffic accident, 2300 block of Lewisville-Clemmons Road, Clemmons. Two-vehicle accident with damages of $5,000 and $2,500.

• Dec. 11 — death investigation, 500 block of Lewisville-Vienna Road, Lewisville. Elderly female was found deceased by her husband.

• Dec. 12 — police service-open door, 2700 block of Lewisville-Clemmons Road, Clemmons. Open entry was found at a local retail establishment.

• Dec. 12 — suspicious person, 8900 block of Lasater Road, Clemmons. Two young adults/juveniles attempted to enter a church.

• Dec. 12 — Contributing to a juvenile, 6600 block of Village Point Lake Drive/Village Point, Clemmons. Suspect snuck juvenile out of home to have sex.

• Dec. 12 — traffic accident, 2600 block of Lewisville-Clemmons Road, Clemmons. Two-vehicle accident with damages of $1,200 and $1,800.

• Dec. 12 — traffic accident, 2500 block of Lewisville-Clemmons Road, Clemmons. Two-vehicle accident with damages of $1,500 and $800.

• Dec. 12 — police service-welfare check, 2500 block of Lewisville-Clemmons Road. Subject thought someone was following him.

• Dec. 12 — police service-welfare check, 7000 block of Garrett Drive, Clemmons. Checked on subject’s welfare.

• Dec. 13 — house breaking without force, 6200 block of Frye Bridge Road, Clemmons. Unknown suspects took $600 cash from victim’s home.

• Dec. 13 — shoplifting and simple assault-non aggravated assault, 2400 block of Lewisville-Clemmons Road, Clemmons. Two females took an item valued at $20 from the Big Kmart without paying.

• Dec. 13 — traffic accident, 2400 block of Lewisville-Clemmons Road, Clemmons. Two-vehicle accident with damages of $2,500 and $1,200.

• Dec. 13 — disturbance/non-criminal, 3600 block of Richland Court, Clemmons. Reporting party involved in a disturbance with an acquaintance.

• Dec. 13 — traffic accident, 1400 block of Lewisville-Clemmons Road/Marty Lane. Two-vehicle accident with damages of $6,500 and $5,000.

• Dec. 13 — employee injury, 800 block of Smoke Tree Circle, Lewisville. Deputy was bitten by a dog while looking for a missing child.

• Dec. 13 — traffic accident, 5000 block of Styers Ferry Road, Lewisville. Two-vehicle accident with damages of $6,000 and $5,000.

• Dec. 14 — found property, 6200 block of Armsburg Road, Clemmons. Laptop bag found by caller’s door.

• Dec. 14 — communicating threats-intimidation, non physical, 6100 block of Frye Bridge Road, Clemmons. Suspect threatened to hire someone to shoot at victim’s home.

• Dec. 14 — traffic accident, 3700 block of Littlebrook Drive/Clemmons Road. Three-vehicle accident with total damages of $17,000.

• Dec. 15 — DWI-provisional/transporting paid liquor/driving while license revoked, 2400 block of Lewisville-Clemmons Road/Ramada Drive, Clemmons. Suspect drove a vehicle after consuming alcohol.

• Dec. 15 — drug violations/paraphernalia, 2400 block of Lewisville-Clemmons Road/Ramada Drive, Clemmons. Suspect possessed marijuana and paraphernalia.

• Dec. 15 — drug violations/paraphernalia/no operator’s license, 1400 block of Lewisville-Clemmons Road/Marty Lane, Clemmons. Subjects found in possession of narcotics during a traffic stop. Cleared for arrest, Crystal Nicole Jones was charged with possession of drugs and Michael Shawn Choplin was arrested for misdemeanor possession of marijuana.

• Dec. 15 — breaking and entering without force/larceny/vandalism, 400 block of Country Lane, Lewisville. Vehicles and an outbuilding were broken into and a chainsaw valued at $200 was stolen.

• Dec. 15 — suspicious vehicle, 1300 block of Thriftwood Trail, Lewisville. Homeowner made report of a suspicious person.

• Dec. 15 — threatening suicide, 100 block of Lewisville-Clemmons Road, Lewisville. Subject was having thoughts of suicide

• Dec. 15 — shoplifting, 6700 block of Shallowford Road, Lewisville. Subject shoplifted a box of condoms at a Rite Aid Pharmacy with a total stolen value of $30.

• Dec. 16 — vandalism, 6900 block of August Drive, Clemmons. Unknown suspect backed car into victim’s yard causing $100 in property damage.

• Dec. 16 — police service-fraud, 6600 block of Gentry Circle, Clemmons. Unknown subject attempted to take money from victim’s bank account.

• Dec. 16 — overdose, 7000 block of Garrett Drive, Clemmons. Subject overdosed on morphine.

• Dec. 16 — careless and reckless driving, 200 block of Lewisville-Vienna Road/Grapevine Road, Lewisville. Unknown vehicle almost struck cyclist.
