Below is a list of crime investigations and activities reported by the Forsyth County Sheriff’s Department in Clemmons and Lewisville last week.

• Oct. 12 — FCSO police service-welfare check, 6900 block of Logans Way Ct., Clemmons. Welfare check of juvenile.

• Oct. 13 — Traffic accident, 2500 block of Lewisville-Clemmons Road, Clemmons.

• Oct. 13 — Simple assault, non-aggravated assault, 1400 block of Sir Charles Dr., Clemmons. Suspect refused to leave upon request.

• Oct. 13 — Drug violations, 6800 block of Morgan Place Dr., Clemmons. Marijuana located during traffic stop.

• Oct. 13 — Suspicious vehicle, 300 block of Riverwood Dr., Lewisville. Subject believed a vehicle in the neighborhood to be suspicious.

• Oct. 13 — Larceny from building, 6700 block of Shallowford Road, Lewisville. Unknown subject stole the victim’s phone.

• Oct. 13 — Abandoned vehicle, 100 block of southbound U.S. Hwy. 421/Williams Road, Lewisville. Vehicle left abandoned on roadside.

• Oct. 14 — FCSO: All other offense — animal related, 2700 block of Park Oak Dr., Clemmons. Dog running at large.

• Oct. 14 — Traffic accident, 6500 block of Rollingwood Dr./Willowood Dr., Clemmons.

• Oct. 14 — Drug violations, 2500 block of Lewisvile-Clemmons Road, Clemmons. Cocaine found during DWI vehicle search.

• Oct. 14 — Police service-assist agency, 4600 block of River Gate Dr./Loop Road, Clemmons. Subject wrecked vehicle and fled scene on foot.

• Oct. 14 — Police service-assist agency, 100 block of Briar Creek Manor, Lewisville. Subject suffering from mental disorder attempted to harm self.

• Oct. 14 — FCSO-police service-fraud, 100 block of Ridgebury Ct., Lewisville. Individual scammed over internet.

• Oct. 15 — Police service, non-criminal call for service, 2600 block of Lewisville-Clemmons Road, Clemmons. Female got into wrong car.

• Oct. 15 — Larceny — all other, 8300 block of Grove Creek Dr., Lewisville. Unknown subject stole signs out of victim’s yard.

• Oct. 16 — Hit-and-run accident, 2200 block of Megan Cr., Clemmons.

• Oct. 16 — Traffic accident, 1700 block of Lewisville-Clemmons Road, Clemmons.

• Oct. 16 — FCSO — locate wanted person, 2400 block of Lewisville-Clemmons Road, Clemmons. Located wanted person.

• Oct. 16 — Hit-and-run accident, 1100 block of S. Peace Haven Road/Harper Road, Clemmons.

• Oct. 16 — Police service-open door, 1500 block of Lewisville-Clemmons Road, Clemmons. Located open door.

• Oct. 16 — Other liquor law violations, 2400 block of Lewisville-Clemmons Road, Clemmons. Subject admitted to drinking under age.

• Oct. 17 — Police service, non-criminal call for service, 1600 block of Harper Spring Dr., Clemmons. Reporting person believes sex offender is violating requirements.

• Oct. 17 — Vandalism, 4000 block of Clemmons Road, Clemmons. Unknown male struck victim’s vehicle several times.

• Oct. 17 — Suspicious person, 2500 block of Lewisville-Clemmons Road/Stadium Dr., Clemmons. Female located walking in the roadway.

• Oct. 17 — Found property, 7200 block of Franklin Road, Lewisville. A bag was located in a driveway.

• Oct. 17 — Larceny from auto, 6800 block of Shallowford Road, Lewisville. License place removed from vehicle while subject was working.

• Oct. 18 — Larceny from building, 1400 block of River Ridge Dr., Clemmons. Suspect took wallet that was lying on the ground.

• Oct. 19 — FCSO — other sex offense-sex offender violation, 1600 block of Harper Spring Dr., Clemmons. Registered sex offender failed to register in county.

• Oct. 19 — Traffic accident, 3500 block of Clemmons Road, Clemmons.
