Below is a list of crime investigations and activities reported by the Forsyth County Sheriff’s Department in Clemmons and Lewisville last week.

• April 12 — traffic accident — 5900 block of Marty Lane/Lewisville-Clemmons Road, Clemmons. Two-vehicle accident with damages of $4,000 and $3,000.

• April 12 — dog bites and attacks — 2600 block of Neudorf Road, Clemmons. Dog bite occurred.

• April 12 — traffic accident, 2000 block of Lewisville-Clemmons Road, Clemmons. Two-vehicle accident with damages of $12,000 and $8,000.

• April 12 — larceny of gas — 4400 block of Hampton Road, Clemmons. Suspect failed to pay for fuel at Carlton’s Grocery.

• April 12 — traffic accident — 2300 block of Lewisville-Clemmons Road/Allegacy Way, Clemmons. Two-vehicle accident with damages of $3,500 and $1,200.

• April 12 — aggravated assault/vandalism, 7000 block of Family Way, Clemmons. Shots fired into occupied dwelling with damages of $1,500.

• April 12 — counterfeiting — 6300 block of Shallowford Road, Lewisville. Subject unknowingly in possession of counterfeit money.

• April 12 — death investigation — 800 block of Grinnell Street, Lewisville. Subject found deceased in residence.

• April 13 — hit-and-run accident — 1400 block of Lewisville-Clemmons Road, Clemmons. Damages of $1,000 to vehicle.

• April 13 — fraud — 1500 block of Kenwalt Drive, Clemmons. Unknown suspect hacked person reporting’s computer.

• April 13 — vandalism — 6400 block of Shallowford Road, Lewisville. Offender spray painted obscenities on rear exterior wall of Utopia Fitness.

• April 13 — vandalism — 1100 block of Norwood Lane, Lewisville. Subject shot the side of the victim’s house with a BB gun.

• April 14 — lost property — 100 block of  westbound I-40/Harper Road, Clemmons. Lost county-issued iPhone valued at $400.

• April 14 — traffic accident — 3500 block of Spangenberg Road, Clemmons. Two-vehicle accident with $3,000 in damage to both vehicles.

• April 14 — traffic accident — 2500 block of Lewisville-Clemmons Road, Clemmons. Two-vehicle accident with damages of $1,200 and $200.

• April 14 — traffic accident — 6000 block of Meadowbrook Mall Court, Clemmons. Two-vehicle accident with damages of $4,500 and $5,500.

• April 14 — simple assault, non-aggravated assault — 3600 block of Clemmons Road, Clemmons. Suspect hit victim in face with a pocketbook.

• April 15 — dog bites and attacks — 2900 block of Cameron Village Court, Clemmons. Cat bite occurred.

• April 15 — traffic accident — 2700 block of Lewisville-Clemmons Road, Clemmons. Two-vehicle accident with damages of $3,500 and $2,000.

• April 15 — autobreaking and larceny — 1400 block of River Ridge Drive, Clemmons. Radio stolen out of unsecured vehicle.

• April 15 — rabies exposure — 5500 block of Linden Hill Lane, Lewisville. Rabies investigation.

• April 16 — automobile theft — 6900 block of Morgan Place Drive, Clemmons. Rental car stolen from apartment parking lot.

• April 16 — traffic accident — 2700 block of Lewisville-Clemmons Road, Clemmons. Two-vehicle accident with damages of $3,500 and $3,000.

• April 16 — runaway — 5000 block of Scotland Court, Clemmons. Juvenile walked away from home.

• April 16 — autobreaking and larceny — 4000 block of Clemmons Road, Clemmons. Unknown subject broke into victim’s car and stole credit cards.

• April 16 — locate wanted person — 6200 block of Towncenter Drive, Clemmons. Wanted person located during traffic stop.

• April 16 — breaking and entering with force — 2200 block of Megan Circle, Clemmons. Unknown subjects forcefully entered into vacant apartment.

• April 16 — suspicious vehicle — 1000 block of Laurel Creek Lane/Shadowbridge Drive, Lewisville. Suspicious vehicle parked at stop sign.

• April 16 — disturbance/non-criminal — 6600 block of Shallowford Road, Lewisville. Subject became irate of his dog’s medical problems.

• April 16 — all other offenses, animal-related — 7000 block of Damon Drive, Lewisville. Found dog.

• April 18 — police service, assist agency — 1700 block of Lewisville-Clemmons Road, Clemmons. Fatal single-vehicle crash.

• April 18 — aggravated assault/discharging firearm — 1400 block of Lake Cottage Road, Clemmons. Subject shot at victim during disagreement.
