Below is a list of crime investigations and activities reported by the Forsyth County Sheriff’s Department in Clemmons and Lewisville last week.

• Dec. 6 — Identity theft — 4000 block of Beaver Brook Road, Clemmons. Subject reported unemployment fraud.

• Dec. 6 — FCSO, police service, civil dispute — 2600 block of Neudorf Road, Clemmons. Dispute over money given someone else.

• Dec. 6 — FCSO, all other offense, animal related — 1700 block of Brandywine Drive, Clemmons. Found dog.

• Dec. 6 — FCSO, harassment, non-criminal — 700 block of Newington Drive, Clemmons. Subject was being harassed.

• Dec. 6 — Larceny, all other, vandalism — 1300 block of Lewisville-Clemmons Road, Lewisville. Offenders took trailer and lawn mower from property.

• Dec. 7 — FCSO, police service, fraud — 6100 block of Hanes Way, Clemmons. Received unemployment paper in mail.

• Dec. 7 — Communicating threats, intimidation, non-physical — 6100 block of Barrington Oaks Drive/Hampton Road, Clemmons. FedEx driver was threatened.

• Dec. 7 — FCSO, police service, fraud — 1700 block of Ramhurst Drive, Clemmons. Identity was used in attempt to receive unemployment.

• Dec. 7 — Found property, 1400 block of River Ridge Drive, Clemmons. Wallet found in parking lot.

• Dec. 7 — Drug violations, violation of auto law, all other — 100 block of northbound U.S. Hwy. 421/Williams Road, Lewisville. Subjects were in possession of cocaine and marijuana.

• Dec. 7 — Police service, non-criminal call for service — 7600 block of Grapevine Road, Lewisville. Individual inquired about the involuntary commitment process.

• Dec. 7 — FCSO, police service, fraud — 1300 block of Meadowgate Lane, Lewisville. Fraudulent use of credit card.

• Dec. 7 — FCSO, police service, information provided — 9600 block of Styers Ferry Road, Lewisville. Unknown subject attempted to obtain Social Security benefits.

• Dec. 7 — Drug violations, driving while impaired, FCSO, K-9 activity — 6800 block of Shallowford Road, Lewisville. Welfare check led to DWI and drug investigations.

• Dec. 7 — Police service, non-criminal call for service — 600 block of Lissara Lodge Drive, Lewisville. Subject reported suspicious phone call.

• Dec. 8 — Death investigation – 6200 block of Cook Avenue, Clemmons. Subject found deceased at apartment.

• Dec. 8 — Counterfeiting, using — 2500 block of Lewisville-Clemmons Road, Clemmons. Business owner found counterfeit money.

• Dec. 8 — FCSO, police service, information provided — 1700 block of Lewisville-Clemmons Road, Clemmons. Student threatened to shoot up school.

• Dec. 8 — Police service, welfare check, 2400 block of Lewisville-Clemmons Road, Clemmons. Subject falling asleep while at work.

• Dec. 8 — Dog bites and attacks — 2200 block of Catherines Way, Clemmons. Owner bitten by cat.

• Dec. 8 — Driving while impaired, overdose — 6800 block of Shallowford Road, Lewisville. Subject unconscious and unresponsive while operating a motor vehicle.

• Dec. 9 — FCSO, police service, fraud — 5900 block of Edgewater Drive, Clemmons. Possible fraudulent activity on subject’s accounts.

• Dec. 9 — FCSO, police service, fraud — 2600 block of Neudorf Road, Clemmons. A false unemployment claim was made.

• Dec. 9 — FCSO, police service, fraud — 600 block of Drumheller Court, Clemmons. A false unemployment claims was made.

• Dec. 9 — FCSO, disturbance/non-criminal — 2300 block of Lewisville-Clemmons Road, Clemmons. Subject did not want to wear mask during hospital visit.

• Dec. 9 — Shoplifting — 3600 block of Clemmons Road, Clemmons. Suspect attempted to take items.

• Dec. 9 — FCSO, police service, information provided — 1700 block of Lewisville-Clemmons Road, Clemmons. Anonymous tip on gang violence.

• Dec. 9 — Simple assault, non-aggravated assault — 1700 block of Lewisville-Clemmons Road, Clemmons. Students slapped teacher’s hand.

• Dec. 9 — FCSO, police service, fraud — 1000 block of Fallbrook Lane, Lewisville. Checked fraud outside jurisdiction.

• Dec. 10 — Misuse of 911 — 7000 block of Mason Place Court, Clemmons. Female subject continued to call 911.

• Dec. 10 — Shoplifting — 2400 block of Lewisville-Clemmons Road. Clemmons. Offender stole beer.

• Dec. 10 — Communicating threats, intimidation, non-physical — 5700 block of Gayray Street, Clemmons. Communicated threats.

• Dec. 10 — Simple assault, non-aggravated assault — 6000 block of Meadowbrook Mall Court, Clemmons. Intoxicated individual placed hand on stranger, then punched in face.

• Dec. 10 — FCSO, all other offense, animal related — 700 block of Dalton Road, Lewisville. Stray dogs.

• Dec. 10 — Dog bites and attacks — 1000 block of Fallbrook Lane, Lewisville. Dog bite during neurological event.

• Dec. 11 — Obtaining money by false pretense — 2300 block of Lewisville-Clemmons Road, Clemmons. Unknown subject obtained credit card information.

• Dec. 11 — Drug violations, paraphernalia, possessing/concealing equipment — 2300 block of Lewisville-Clemmons Road, Clemmons.  Offender possessed marijuana and scale.

• Dec. 11 — Overdose — 7600 block of Yadkinville Road, Lewisville. Overdose.

• Dec. 12 — Police service, non-criminal call for service — 3900 block of Clemmons Road, Clemmons. Report of possible neglect.

• Dec. 12 — Misuse of 911 — 7000 block of Mason Place Court, Clemmons. Female subject continued to call 911.

• Dec. 12 — Overdose — 7900 block of Abelia Way, Clemmons. Male ingested unknown amount of unknown substance(s).

• Dec. 12 — FCSO, police service, security check — 6600 block of Gentry Circle, Clemmons. Subject called to report vehicle stolen, no contact could be made.

• Dec. 12 — FCSO, all other offense, animal related — 1000 block of Lewisville-Clemmons Road, Lewisville. Stray dog.
