Below is a list of crime investigations and activities reported by the Forsyth County Sheriff’s Department in Clemmons and Lewisville last week.

• Feb. 14 — Breaking and entering without force — 7000 block of Mason Place Court, Clemmons. Apartment broken into and vandalized.

• Feb. 14 — Simple assault, non-aggravated assault — 6300 block of Cephis Drive, Clemmons. Unknown subject assaulted patrons at bar.

• Feb. 14 — Auto-breaking and larceny — 900 block of Brookway West Drive, Lewisville. Items taken from unlocked vehicle.

• Feb. 15 — Dog bites and attacks — 3400 block of Tanglebrook Trail, Clemmons. Cat bite.

• Feb. 16 — FCSO, domestic disturbance — 6700 block of Forest Oak Drive, Clemmons. Reported as assault, unable to determine if the same occurred.

• Feb. 15 — FCSO, police service, information provided — 1100 block of Lewisville-Vienna Road, Lewisville. Student stated he was going to kill himself.

• Feb. 16 — FCSO, police service, fraud — 600 block of Schaefer Court, Clemmons. Unknown suspect(s) attempted to take out loan in reporting party’s name.

• Feb. 16 — Police service, welfare check — 6600 block of Gentry Circle, Clemmons. Anonymous caller stated juvenile made suicidal comments.

• Feb. 16 — FCSO, police service, information provided — 1400 block of River Ridge Drive, Clemmons. Reporting party thought a subject looked like a runaway juvenile.

• Feb. 16 — Shoplifting — 1400 block of River Ridge Drive, Clemmons. Subject took items from the store without paying.

• Feb. 17 — Shoplifting — 1400 block of River Ridge Drive, Clemmons. Offender concealed merchandise under shirt and left store.

• Feb. 17 — Auto-breaking and larceny — 4000 block of Clemmons Road, Clemmons. Suspect smashed window and took personal property.

• Feb. 17 — FCSO, police service, fraud — 2900 block of Hopewood Lane, Clemmons. Reporting party gave personal information to unknown suspect(s).

• Feb. 17 — Police service, non-criminal call for service — 2600 block of Lewisville-Clemmons Road, Clemmons. Subject requested to be taken to the hospital.

• Feb. 17 — FCSO, police service, information provided — 100 block of Ridgebury Court, Lewisville. Subject vehicle was repossessed by bank.

• Feb. 18 — Auto-breaking and larceny — 4000 block of nature Trail Drive, Clemmons. Unknown person(s) entered unlocked vehicle and took items.

• Feb. 18 — Automobile theft — 7300 block of Riverview Knoll Court, Clemmons. Suspect stole vehicle from parking lot.

• Feb. 18 — FCSO, harassment, non-criminal — 100 block of Brightstar Lane, Lewisville. Individual reported harassing calls.

• Feb. 18 — Police service, non-criminal call for service — 6400 block of Ferry View Lane, Lewisville. Subject reported she thought she had been poisoned.

• Feb. 19 — Trespassing — 3600 block of Clemmons Road, Clemmons. Homeless subject on property.

• Feb. 19 — Communicating threats, intimidation, non-physical — 3100 block of Gammon Lane, Clemmons. Disturbance over road rage incident.

• Feb. 19 — FCSO, police service, fraud — 6700 block of Shallowford Road, Lewisville. Unknown subject frauded Subway worker.

• Feb. 20 — Breaking and entering with force — 1500 block of Lewisville-Clemmons Road, Clemmons. Suspect forced window in an attempt to steal cash register.
