Below is a list of crime investigations and activities reported by the Forsyth County Sheriff’s Office in Clemmons and Lewisville last week.

• May 1 — FCSO disturbance/non-criminal — 4100 block of Hampton Road, Clemmons. Female wanted assistance getting away from a male.

• May 1 — Police service, 24 hour lock up — 6200 block of Brewer Avenue, Clemmons. Twenty-four hour hold.

• May 1 — Police service, welfare check — 6300 block of Allegacy Way, Clemmons. Attempted gift card fraud.

• May 1 — Shoplifting — 2600 block of Lewisville-Clemmons Road, Clemmons. Offender stole clothing items.

• May 1 — FCSO, police service, fraud — 1000 block of Glen Day Drive, Clemmons. Person notified by bank of possible fraud.

• May 1 — FCSO, police service, fraud — 100 block of Ballyhoo Drive, Lewisville. Individuals trying to notarize card in another state.

• May 2 — Larceny from auto, vandalism — 6900 block of Harper View Court, Clemmons. Subject had a window broken and engine parts stolen.

• May 2 — FCSO, locate wanted person — 1500 block of S. Peace Haven Road, Clemmons. Wanted subject located.

• May 2 — Locate wanted person — 1500 block of S. Peace Haven Road, Clemmons. Wanted subject located.

• May 2 — FCSO, police service, information provided — 900 block of Dalton Road, Lewisville. Subject stated she was confused about where to place garbage cans.

• May 3 — FCSO, police service, fraud — 2600 block of Lewisville-Clemmons Road, Clemmons. Person reports attempted fraud.

• May 3 — Shoplifting — 2400 block of Market Center Drive, Clemmons. Three females took stuff from Five Below.

• May 3 — Shoplifting — 100 block of Westwood Village Drive, Clemmons. Three females stole an assortment of merchandise.

• May 3 — Simple assault, non-aggravated assault — 6000 block of Meadowbrook Mall Court, Clemmons.  Intoxicated patron attempted to assault another.

• May 3 — Police service, non-criminal call for service — 200 block of Wyntfield Drive, Riverwood Drive, Lewisville. Caller reported child with firearm near lake.

• May 4 — Violence city/county ordinance-firearm related — 3600 block of Richland Court, Clemmons.  Suspect discharged a firearm into the ground.

• May 4 — FCSO police service, civil dispute — 6200 block of Ramada Drive, Clemmons. Dispute regarding money.

• May 4 — Dog bites and attacks — 3800 block of Old Rosebud Court, Clemmons. Dog bite.

• May 4 — Police service, open door — 3600 block of Clemmons Road, Clemmons. Located open door to business.

• May 4 — FCSO, police service-civil dispute — 6200 block of Ramada Drive, Clemmons. Dispute regarding money.

• May 4 — Dog bites and attacks — 3800 block of Old Rosebud Court, Clemmons. Dog bite.

• May 4 — Police service, open door — 3600 block of Clemmons Road, Clemmons. Located open door to business.

• May 4 — FCSO, police service-information provided — 300 block of Meadowbrook Lake Drive, Lewisville.  Subject concerned about his firearm purchase online.

• May 4 — FCSO disturbance/non-criminal — 1300 block of Lewisville-Clemmons Road, Lewisville. Male caused a disturbance upon requesting a refund.

• May 4 — Police service, welfare check — 6500 block of Shallowford Road, Lewisville. Woman suffering from cognitive impairment was reunited with her daughter.

• May 5 — Trespassing — 6000 block of Meadowbrook Mall Court, Clemmons. Intoxicated patron refused to leave upon request.

• May 5 — Communicating threats, intimidation, non-physical — 2400 block of Lewisville-Clemmons Road, Clemmons. Subject threatened to shoot victim.

• May 5 — Simple assault, non-aggravated assault — 3000 block of Cameron Village Court, Clemmons.  Person alleged assault from boyfriend’s girlfriend.

• May 5 — FCSO, police service-information provided — 1100 block of Lewisville-Vienna Road, Lewisville.  Student believes the saw a threat on a phone.

• May 7 — Death investigation — 100 block of Willow Trace Circle, Clemmons. Death investigation regarding possible OD on prescription.

• May 7 — FCSO, violation of auto law-equipment violation, no operator license-violation of auto law, no insurance-violation of auto law — 100 block of westbound I-40/Lewisville-Clemmons Road, Clemmons.  Traffic stop yielded no operator license.

• May 7 — FCSO disturbance/non-criminal — 1400 block of River Ridge Drive, Clemmons. 

• May 7 — Auto-breaking and larceny, larceny-all other — 6300 block of Clemmons Point Drive, Clemmons.  Offender stole keys, unlocked vehicles and used stolen credit cards.

• May 7 — Auto-breaking and larceny — 5700 block of Woodside Forest Trail, Lewisville. Victim had phone charger stolen from vehicle.

• May 7 — Automobile theft — 200 block of Bradford Lake Court, Lewisville. Two masked males hotwired vehicle.
